As soon as the brochure for the seminar is available, please send 3 brochures to 2219 Ryons St., Lincoln, NE 68502. I will begin the handout as soon as I receive the brochure so that I can be sure to include everything that is advertised in your brochure.

PAYMENT
ALL honorariums are to be paid on the day of the seminar. Please have the check ready once the seminar has been completed.

Videotaping and /or audio taping will be additional unless discussed with Nancy. If the tapes are to be used for in-house educational opportunities, the fee for videotaping will be $125.00 and the fee for audio taping will be $55.00, regardless of the length of the seminar. If the tapes are to be sold to the participants, the fees will be negotiated prior to the speaking engagement. A complimentary copy of the audio or videotape will be provided for Nancy after the seminar is completed and the tape has been edited.

Expenses will be estimated for each group prior to the speaking engagement and a written statement will be faxed to each group. A partial payment of $500.00 for expenses should be provided with the honorarium check. Any additional expense payment can be mailed to Nancy after she submits receipts for the final expenses. Expenses will include airfare (coach), ground transportation in the host city, hotel room and meals (per diem fee of $45.00 per day). Should transportation delays and/or cancellations necessitate an additional night in the host city, the host will be responsible for the additional payment. Nancy will mail all receipts within two weeks of the seminar.

The fee for canceling a seminar is $100.00 if the cancellation occurs within 4 weeks of the seminar. This fee is also negotiable and will be waived if the seminar is rescheduled in the future. Should it be necessary for Nancy to cancel a seminar, adequate compensation will be provided in the form of a reduced honorarium when/if the seminar is rescheduled.

My federal EIN is 20-1272603 for your accounting and tax purposes. Please have forms available for me to sign at the seminar (Tax forms, etc.).

EQUIPMENT NEEDS
Please inform Nancy of the classroom layout. Equipment needs will vary with the topic or character of the lecture. But generally, Nancy uses a Powerpoint presentation format and requires a laptop computer with LCD projector. She will bring the presentation on a memory stick device to be inserted into a USB port on the computer.

HOTEL
When making Nancy’s reservation, please reserve a NON-smoking room and confirm the room for late arrival. Please have the room and meals direct billed to your group or institution. Nancy will pay any phone charges or any other incidental charges with her credit card.

Please e-mail Nancy the hotel name, address and phone number as well as whether or not the hotel provides ground transportation to and from the airport. Please provide a home phone number of a contact person that Nancy could reach the evening prior to the seminar just in case problems arise with travel or illness.
